With the Horizon TV platform kit you have to look for the cable operator. Liberty Global have registered the kit under the Ziggo operator name (the Netherlands cable operator they launched the platform with) so you need to try any codes starting with Ziggo.

Did you ever get this to work? I'm hitting the same issue with the same model box.

Ziggo, also Ziggo Mediabox Next both work on the 360 boxes which uses the same Horizon system, although it can take 3 or 4 attempts before it's successful.
You can use some third party remotes such as the OneForAll URC7125 - use the SimpleSet setup process and choose Ziggo manufacturer. There's also the OneForAll1240 - if you enter code 1582, It's not as expensive as the 7125 and has 4 selects for tv/sat/amp/dvd. The OneForAll remotes should also be able to work with your Samsung TV. If you want to try one of these remotes then I suggest buying them from Amazon as you should be able to return the remote if it doesn't work.
There's no guarantee the following will work, but it did work on the 360 box.
Try selecting satellite box > tv directly
Then enter your postcode and select Virgin
Select the HDMI port that your Virgin box is connected to and hit Channel Up and select NO around half a dozen times. After that it will offer you to search for model of your box manually. Try VIP5002W (or whatever is printed on the base of your Stream box, also try Arris DCX960 or just DCX960, and also Pace DCR7111
