Maximum Devices Reached - 1 device says this everyday then locks out for a month!

as I said, every single time I login, it asks for a new device name, then hey presto, we're up to five and I can't login the next time on the same device.

Last happened a couple of years ago and someone helpful in here managed to reset it for me.
