I didn’t receive a survey

I just had a call with a guy from customer service who is on trail. He massively helped me with my package. However I never received a survey message on my phone. Is there a way I could find out who I was talking to so I can write up a good summary about how helpful he was? He was really helpful and I will feel guilty if I can’t fill out his survey form for him. 

hoping someone can help me identify the individual!!
