Phone adapter

Hi, I have seen you supply a phone adapter on these forums. I am having a problem with mine, it keeps falling apart. Can you help me get a replacement 

RJ11 to BT Plug Adaptor - Connect ADSL DSL Cable to BT Telephone Phone Socket

    Just seen it is a no reply on the email address.

    Yes it was supplied and installed when our phone was changed to the ip protocol. I have just installed a new router and the phone wouldn't work, it was due to this plug being faulty. Instead of calling out an engineer I thought I would see if I could just request for a plug.

    Sorry to hear of your concerns with your adapter. You say it's falling apart. Is this an adapter that we supplied? 
    Are you able to take an image of this, so we can investigate further?
