The Hub5 is available to new higher tier customers. There is no way that existing customers can ‘request’ one, you will be provided with the appropriate hub for your tier, it may even be a second hand one.

I’ve been trying to get one for ages because the Hub 4 has stopped working properly. They’ve said I can upgrade to a Hub 5, but I have to pay a £35 “Activation Fee”.

Not really.
You don't 'need' a HUB 5 to get full speeds across several devices as advertised, the HUB 4 is more than capable.
