Termination fee credit

Hi, today I've cancelled my contract as I'm moving house and I'm not taking the services with me. During the online chat, I was told that I was to send in a copy of my new rent agreement (that shows new address) , so a credit for the (pro rata) termination fee can be credited to my account  for the final bill.

Can someone point me in the right direction on how & who to send this to? 

I did screen shot but happened to miss that bit 🤦🏻‍♂️
