Not my account in arrears.. credit score affected

I recently checked my credit score and was concerned to see a Virgin Media account with my name but not my current address. 

This account is in arrears and negatively affecting my credit score. 

Please can I have some advice on how to take my name of this account and how to correct my credit score.

Thanks to Beth_G my credit score looks to have reverted back to it's original position and the negative events on my credit account have been removed. I did also have to raise a dispute with Equifax after flagging with VM.

I will keep an eye on it to make sure this is correct but it seems that this is the best place to actually get anything done as opposed to going on the phone.
