Sim ordered fraudulently

Today I was on a webchat with "dominic" regarding a contract renewal offer. The chat got disconnected. Tonight I have received an email from O2 regarding my sim order! I want this order stopped as I did not order this and am very upset at the underhand way this has happened. I also want to make a formal complaint if anyone could point me in the right direction to do this. Thanks

    Thank you for your post. We're very sorry to hear about the SIM order you've had that you didn't request. 

    We can certainly investigate this and look into what/why this has happened.

    I thought this had all been sorted out after I contacted o2 to cancel.I had an email from them yesterday confirming immediate disconnection but tonight I have received an email with my final bill of £4.68. Will have to get back on to o2 now to have this charge removed. Wish I'd never got in touch with virgin!
