Default on credit report

Hello I left virgin media in May this year I had a final bill for the sum of £69.34 in June which has been paid but you have put a default on my equifax why has this happened and can you remove it.

    I was speaking to an adviser 3 days ago. I have responded numerous times, and no one is getting back to me can someone help
