Change of name form

Hi I need to change my surname due to getting married 7 years ago just didn’t bother doing it I’ve been sent a form to fill out by email but I can’t fill it out it won’t let me type, what should I do? Thanks

The form is password protected and all but the first data entry fields are locked.

Try opening it in Wordpad (or another simple text editor)

It will give you the basic form minus the VM graphic at the top.

Only VM would send out a form to complete which can't be completed electronically!

The form also seems to show a return postal PO box address for VM in Swansea which, I believe, is no longer operational and contradicts the address shown below

Thankyou I did phone them to explain and my details were changed over the phone, So hopefully my surname will get updated.

Hi Carley, would you be able to help with updating last name after marriage and a forst name error done by Virgin when i purchased the contract? I have sent the change of name form at leat 5 times and NEVER got any reply , also i called and they lied is done bit ia not. I have got the marriage certificate to show the change... i really need this to be updated asap
