VM's EPG data is supplied by the broadcasters, and processed into the correct format before being released for use. It's not unusual for broadcasters to supply the wrong data, and unless this is picked up before broadcast, it will stick.
If you spot errors in advance of broadcast - by comparing between the TV guide & the broadcaster's own schedule - you can report them to tivoguideerrors @ virginmedia.com (lose the spaces thanks to a current bug with this forum) who won't generally reply but will put a poke in the right direction.
In this case where it's now happened, the only answer is "it happens". A V6 lets you use "search & explore" to either find alternate showings, or view from OnDemand.

It failed again today even though the EPG was correct. It said "duplicate"

TiVo recording logic is that once an episode is listed in your recordings, it will not be duplicated unless 1: More than 28days (might be 30) have passed since the last recording, and 2: The SL is set to record "everything"
If either of these criteria fail, then TiVo is designed to drop the recording as you already have it. I can't recall exactly how "New & Reruns" performs in this scenario, but am fairly sure the recording is still dumped.
