Sorry to see you are having major issues with your new coonection. It looks like VM have sent a replacement kit instead of a new starter pack.
I hope this helps. It looks like you need to speak to New accounts and Set Up for them to send out the missing equipment.
That would be options 1,1,1 and 4 if dialling 150 from a Virgin landline , or options 1,1,2 and 4 if dialling 0345 4541111 from any other phone. If calling it's best to call around 8am as lines are least busy then.
Or you can wait for a member of the Forum Team to pick this up for you, however it may take a day or two.
