Recording issues

My 360 box is not recording planned recordings. The programmes appear in my saved programmes but with an error logo and there is no actual recording. Also older recordings are not playing back.
Have rebooted but still same problem.
Anyone else had this problem? Is this a hard drive failure?

Fixed.
Factory reset but not re-format option worked
