proof of usage

good morning

recently i claimed my mobile, so the  insurance company  requested submit a proof of usage. which helps me to claim a phone.

Your monthly bill would be classed as a proof of usage of the phone over our network.

But the thing is insurance company is not accepting that and they are requesting as follows
Proof of usage is a document which can only be obtained by contacting your network provider. please
note this isn’t a copy of your monthly bill.
Any evidence must contain:
• Letterheaded document
• Name and address of the company issuing the document
• Customer Name
• Customer Address
• Mobile Number
• IMEI and/or Serial Number of the device
• Date of First Usage
• Date of Last Usage
