Had a new virgin SIM card and have no caller I’d and duplicate telephone numbers one with spaces and one normal

Would you please be able to confirm a few extra bits of information for us:

1) The make and model of handset you are using

2) When you check the settings on your mobile is the number listed correctly? 

3) When you check the settings on your mobile is no-caller ID enabled?

4) Are you able to test the SIM in another handset, and if so does the problem still happen?
