My wife's phone has recently had these problems:
1. No 4G data
2. Cannot make outgoing phone calls
Mobile data is turned on and the APN settings are correct. She can receive phone calls and can send/receive texts.
I have swapped her SIM with mine and the problems followed the SIM card, not the phone.
How do I order a replacement as there appears to be no online way of doing so?

Really sorry to hear that your wife's SIM is having a few issues with her SIM, this isn't good.
Have you checked the online account to ensure all payments are up to date and that the credit limit hasn't been reached?
If everything is fine then we can certainly have a closer look at what is going on and sort out a replacement is needed,
