New SIM card for Oomph bundle

I signed up for the oomph bundle back in 6 October. A technician came on 20th to setup my TV, Internet, and Phone line. Works perfectly, but I have not received the SIM card yet. Who should I to to request the SIM card? And as I'm switching from Three, how would I go about to keep my number?

Really sorry for the delay in getting back in touch and that you've not had your SIM in the post, this isn't good.
