My contract is for 3 years including a phone. Phone is broken. How can I find out if it is insured? Can Virgin send me a new one?

You can make a claim yourself have a look at https://www.virginmedia.com/help/virgin-media-protect-faqs
Just to note that before the insurance will send a replacement phone they would need you to send the phone to them for assessment to see if can be repaired.
