"1GB" connection never gets over 40-50MB

We upgraded to the 1GB service a few months ago, and while it start OK-ish - speeds of 500-600MB - in the last few weeks it's rarely gone over 40-50MB, and often a lot less, plus lots of dropping out etc. We've done all the obvious things - hard reboots etc - but got to the point where we're tearing our hair out because we need superfast broadband for work purposes, and it's hard to see exactly what we're paying the premium for.

What are the speeds like on a device connected on a known and good Cat5e/6 ethernet cable?

What are upload speeds like?

I do not work for VM. My services: HD TV on VIP (+ Sky Sports & Movies & BT sport), x3 V6 boxes (1 wired, 1 WiFi, 1 on PA) Hub5 in modem mode with Apple Airport Extreme Router +2 Airport Express's. On 250Mbps, Talk Anytime Phone, x2 Mobile SIM only iPhones.
