Several of your Downstream Power Levels are too low.
This will need an engineer to put right.
You can either call in (08:00 is the best time) or wait on here for a few days (6 to 10 days average) until one of the VM Forum Staff comes along.
If you wait on here Do Not bump the thread it will put it further down the VM 'to do list'.

One of the upstream QAM levels is off (All should be 64)

Saves them work !!
If it doesn't matter why have them at 64 QAM in the first place ?
The serious problem is the wide variation of downstream signal levels. When that is sorted it will very likely affect the upstream too.
How it affects you depends on how the noise affects your signal. If the lower modulation means that the codewords get through without being lost then your problem lies elsewhere.
