Direct debit fraudulent set up

I have had a direct debit set up on my bank account to NEWASURION EUR RE TESCOPHONEINS I know nothing about this and have cancelled the direct debit immediately some one has used my details to set this up but was not me.

why has this happened I notice it has happened to others

The reference you have posted appears to be to do with Tesco Mobile phone insurance. How does this relate to VM?
