I am having the same issue, and frequent disconnects for the last 2-3 months.

Faults or Outage
Check for a known local fault on 0800 561 0061 - it is an automated service
The lack of a Locked upstream channel suggests a service fault.
The massive number of errors on the Downstream channels also points to a service fault.
