Damaged Catv manhole

i have a catv manhole on my driveway that is damaged - can someone advise me who to contact

Add a picture of the damaged CATV cover to give an idea of what is to be fixed.

That looks a very old manhole cover which might not have a direct replacement. They might find it helpful to see a photo of the whole cover in its surrounding surface.

That's a pavement cover and is probably not meant to be driven over.

All sorted today !! Much stronger solution than origin.
