Received Gone Away form received

Hi Team,

My broadband order was supposed to be delivered today but got a mail a few days back that my gone away form was received. Later didn't get any follow up mails regarding the status of my order. I haven't been informed about my account number, without which I'm unable to contact the customer care. 

Hoping to get some help on this.
