I forgot to add the repair form in with the return packaging with all the details on it, what shopuld I do?

Just to confirm, did you return all the relevant equipment in the packaging? Also when was this returned to our team? Have you had any correspondence regarding the returned equipment since you sent it off?

I sent the phone inside the return box just forgot to put the form in I took a screenshot and emailed it

Thank you for reaching back out, I have spoken to a colleague who has advised there should be an issue as the IMEI number will link the phone to you which would have been inputted when booking the repair in.

There shouldn't be an issues as the IMEI will link the phone to you.
