If you're looking to obtain a Proof of Purchase for a handset that you purchased from us, please complete and return this form to us. The form details how and where to send it back to us once completed.
If you didn't purchase the handset from us then we won't be able to provide you with Proof of Purchase. Instead, you'll need to make contact with the company/seller you purchased it from.
Are they still doing this now that o2 have taken over I've tried to contact Virgin I've sent a form off but not had a response I need it for an insurance clame I don't seem to be getting anything back
