Unable to register on My Virgin Media App

My Virgin equipment was installed on 11/06 and since then I've been unable to register myself on My Virgin media. When I click on the Register button a screen is displayed below. I've rung 03454541111 numerous times but no one seems able to resolve this issue for me.
