Rolling monthly discount-have to chase every month

I agreed a monthly £15 discount on my broadband last September.

However, every month it's not applied to my bill and I have to chase it. Every time I do so, I am assured that it will be added automatically from then on but it never is.

How can this be resolved for good? (and verified?)
