Damaged Virginmedia green box

I want to report a Damaged Virginmedia street box (green)

postcode HA6 2AX at junction of Linksway and the Broadwalk. Northwood

the online form took ages to fill in. When sent it said ‘Oops. Page not active’

HOPELESS!!!!!

I am sorry to hear that the cabinet has been damaged, do you have a picture of cabinet or can you explain how to cabinet is damaged so that this can be reported correctly.

please just send someone to have a look

as stated. The front panel has broken off and is on the ground. I picked it up and propped it against the box. 
I contacted BT initially but they say it’s your box
