My wife and I have had email accounts with Ntlworld for the last c20 years - for some reason neither of us can now log in - have we lost these forever?

This depends entirely on whether or not you are still VM broadband customers. If not, then at best, there is a chance, albeit a slim one, that temporary (and I do have to stress temporary) access might be restored to give you a chance to backup and save any emails you want to keep. If you aren’t customers, then, yes, I’m afraid it’s almost certain that the mailbox and emails have gone for good.
