Sent phone in for repairs, sent back not repaired...

Sent the phone out on Friday 8th and got it back today the 13th

Letter they sent had this message

Have you sent us the the correct phone?

The IMEI number and model (D405N) on the paperwork both matches the phone, what occurred? How was it not the correct phone? perhaps the Support agent on the phone logged the wrong details? Is there anyway to see what they logged? or does this paperwork I have tells me the details they logged? 

Has this ever happened to anyone else? Because now I have to enjoy another hour on the phone to support...


Also why was the phone cold? Felt like it came from the fridge.
