Upgrade Letter - can't upgrade online or by phone

I received my upgrade letter today... tried to do it online and got the error message... can't call in as I cannot find my password (which I appreciate is totally my fault)... can anyone help me progress the upgrade?

It can take up to a week for someone from virgin media to reply
