When watching direct TV via 360 box generally picture quality is good, some pixelation on adverts.
When watching catch-up or recorded programs issue are much worse and very pixellated
HD programs do not look like the quality of picture before 360 upgrade

There's a few different bits mixed up here.
Live TV uses the co-ax cable feed, and if that is pixellated tends to indicate a signal fault.
Recorded & paused/delayed TV uses the hard drive, if that breaks up then HDD issues are the likely cause.
OnDemand & streaming uses the internet, pixellation will often be the HDMI lead but then will affect all video sources. Internet connectivity issues result in slow speeds which manifest as buffering & stuttering pictures.
