Refusal letter for phone usage and phone barring

Hi there,

I need phone usage and barring letter for insurance.  But virgin refused it. And now insurance asked for refusal letter which one of team member told me that they can provide and now refusing it. Can someone  suggest me I can have it or not anyone of them.  Please help me with it.
