Airtime voucher not working

I bought an airtime voucher topup from Tesco last week (like I normally do) but every time I try to top up my phone with it I get the following message:

"Sorry but we're having a few technical problems and can't do that for you at the moment.  Please try again later.".

I get this regardless of whether I phone 789 and input the code via the keypad or text it to 789111 (which always worked previously).

Any ideas on what is going on?  Is there a network problem preventing top-ups from being added, or is there a problem with the voucher?
