Data Subject Access Request Form Broken

I’m looking for some help. 

I'm trying to submit a DSAR but every time I click submit on the form it fails. I’ve changed various aspects of my answers to see if it will help but to no avail. 

I’ve also tried it on various different browsers and my mobile, again no luck.

I’ve tried the live chat and they were useless and I can’t seem to get anyone to speak to when I call up.

Are you using commas in the very last free text section of the form?

"Is there any additional information you require?"

I have a past recollection that using commas in that free text box might stop the 'Submit' button from appearing.

Some other cells on the form seem to be "required" but don't tell you this first of all you have to look for red text underneath after a submit failure

We're sorry to hear you're having issues submitting the request. As mentioned by goslow, there are some parts of the form that are mandatory. After getting the error, please go down the form and expand each section and look for the red text. Once you've corrected any errors, this will allow the form to go through.

Funny enough, I had tried to get returns kit for my Hub yesterday, and that error message is exactly the same when I tried to use the returns request form  https://www.virginmedia.com/help/virgin-media-return-or-recycle-equipment/request-returns-pack?

It is the exact same banner, colour and wording .... just odd really.

Anyway, I managed to get another link for the Yodel label from a kind MOD here, so all good now.
