That would be difficult given the result of submitting that form is:

"There was a problem submitting the form."

Yes, I'm aware that you linked the form to individual customers' email address, because the form was pre-filled with all of my personal information (meaning it is a security risk because anyone who has the link - for example by reading the email - or could forge a link, could gain unauthorised access to that information).

My address is the same address linked to my "community" account here, if that helps.
