You have a connection issue on the upstream channels. All the power levels are very high and one channel has dropped to 16 qam (from 64) and you have T3 timeouts on that channel... it is struggling.
You will need to call in and get VM to look at the connection overall probably needs a tech visit. Try at 08.00 if you cant get through today
