we have an original super hub, which has finally given up the ghost and stopped functioning all together. Therefore we would like to know how we go about getting a replacement.

Welcome to the forums, I am sorry to see you have had trouble with your hub.
I have taken a look and your modem SNR is too low and the power levels are out. I would like to get an engineer out to take a look at this for you.
