Your minimum term may end on Dec 5th, but unless you explicitly give 30days cancellation notice, your contract certainly won't - and will continue regardless.
New customer offers require you to be a new customer, which means your existing account needs to be totally disconnected for a period of time. The length of that time can vary, but is measured in weeks/months. You'd need to return your existing equipment and be given new kit when you re-connect.
