Bills - covers what period?

Does the bill cover the future or the past i.e. I get a bill on 15th November, is that for 15 Oct to 15 Nov or 15 Nov to 15 Dec?    [MOD EDIT: Subject title changed for clarity]

This should be shown on the top of the PDF or online bill.. as per the example below..
