Phone is not working [again]

No dial tone since 04/08/2012 morning, internet didn't work for 2 days [02/08/2012-03/08/2012] neither but is working again now.

 

Would like proper help on this on my first visit rather than my 7th, considering I have been two months without my phone working at all with different engineers being sent over the two months.
